This vital field role entails direct patient care and case management within the community, aiming to optimize healthcare outcomes. This position offers competitive salary, on-call stipends, and mileage reimbursement. The final hiring decision will be made by our client agency. Key Details: Salary: $85,000 - $95,000 Per Year, DOE Benefits: On-call stipends, Mileage reimbursement Type: Field role Hiring Decision: Made by client agency Responsibilities: Assess patient conditions and develop appropriate care plans. Maintain comprehensive medical records and report patient status to necessary parties. Act as the primary point of contact among patients, their families, and healthcare providers. Requirements: License: Active RN license in Pennsylvania with no restrictions Experience: At least 1 year in a similar in-home healthcare role.
